Symptom narrative

Vaccination given with Covid 6m-4yr vial that had been mixed the day before on 04/23/2025. Verified the date on the sheet with the vial was 4/23/2025, 3 doses were mixed, one was given and verified on 04/23/2025, the next dose was marked and verified as given on 04/24/2025. Nurse manager notified immediately after it was noticed, and a few hours after vaccine was administered. Provider was made aware of the incident and notified family. Protocol was updated to waste vial every night before end of shift to ensure it can't happen again in the future.
