Loss of consciousness

Symptom narrative

Patient passed out when checking out from her visit. Patient was observed for 10 minutes in the exam room post vaccine. Patient regained consciousness quickly and was assisted to a wheelchair. Patient was laid on exam table in supine position with head of bed at approximately 30 degrees and given juice to drink after sitting up further.
