Symptom narrative

Patient came into office on 4/28 for Rash - Appeared on his arm 3 days ago after a physical exam where he had received Hep A - Worsened progressively, especially with heat - Treated with Benadryl and Tylenol Skin: red round rash all over face, chest, back, arms and legs- urticaria multiforme

Current illness

Was on Amoxicillin for Pneumonia- was seen in office on 4/17 and sent for blood work and cxr and found (+) for pneumonia on 4/18, antibiotic started

Other medications

Amoxicillin 400mg/5ml

Lab data

For urticaria multiforme Would use zyrtec 1/2 teaspoon in the morning And benadryl 1 teaspoon at night Pepcid twice a day If joints get swollen, / having a hard time walking, will need to add prednisone Referral to allergiest to test him to for amox allergy which I think is the likely culprit
