Symptom narrative

During a pillbox appointment, I administered 2nd dose of Shingles vaccine. While drawing up the diluent, I thought I had injected into the lyophilized vial to reconstitute but I believe I injected back into the same diluent vial. After that, I left the room to try and find a smaller needle for administration into the patient. I injected the diluent into the patient, who commented on a "burning sensation" which I attributed to a normal response to Shingles Vaccine. After patient left, I observed the un-reconstituted vial on the counter.
