Symptom narrative

The patient received three vaccines. Shortly after administration (around 5 minutes), patient felt dizzy and sat down on benches near customer service. She then passed out and slipped onto the floor. The patient lost control of bladder but quickly regained consciousness. The life squad was called for evaluation, and the patient went with them to the hospital to be further checked out.
