On 25-APR-2025, the patient was inadvertently administered dose of expired hepatitis A vaccine, inactivated (VAQTA) (0.5 ml/dose; lot # X025908, expiration date: 24-APR-2025; strength, formulation, dose #, route and anatomical site were unknown) for prophylaxis (expired product administered).
