Symptom narrative

Student was administered unnecessary doses of Pediarix (Dtap, Hep B, Polio). Nurse completed the record review of student and missed a vaccine record in student education profile. the vaccine record was not entered into database or EHR. Nurse then entered vaccine records from profile into database and EHR. the Dtap vaccine was flagged "age too young," Hep B, Polio, and MMR was not flagged, however they were extra doses.
