Symptom narrative

My daughter had her vaccine yesterday and complained that her bone hurt a few hours later in her left arm. As a medical assistant myself, I looked at where the injection site was, and it was too close to the shoulder. The placement was too high and too close to shoulder. Patient woke up this morning with the pain being worse than the day before.
