Symptom narrative

Significant redness and swelling at vaccine site in r thigh the day after vaccine. Parent went to an UC, pt was given antibiotics. No documentation of this available to me as was an outside facility. Sounds like a large local reaction. Did see her in office 5/5/23 for diffuse full body macular rash developed 5/3/25. No fever or other symptoms.
