Child was very anxious/combative, kicking, fighting. Grandmother, his aunt, and 2 nurses Attempted to give Kinrix vaccine IM to R deltoid, pt jerked his arm away pulling the syringe out of his arm and bending the needle and scratching his arm in the process. Scratched area was cleaned with an alcohol pad and a band aid was applied. Grandmother was instructed to schedule an appt with his PCP for his immunizations. Grandmother states she has concerns about his behavior-states he is out of control and combative most of the day. She states she has plans to have him evaluated.
