Symptom narrative

Patient was given HPV (Gardasil 9) on 04/29/25. Patient noticed the start of hives on 04/30/25. Started taking OTC Cetrizine to help alleviate the rash. On 05/03/25 patient and his mother went to ER due to the hives. Patient was given Prednisone 20 mg BID x 5 days. Seen by PCP on 05/06/25 and hives are still present on B/L arms, legs, and on patients face.

ADHD, Exercise induced asthma, Anxiety

Other medications

Methylphenidate 40 mg daily; Cetirizine 10 mg daily as needed
