hypertension, DVT bilateral lower extremities, fibromyalgia, type 2 diabetes mellitus, gastroesophageal reflux disease, PTSD, chronic obstructive pulmonary disease, bipolar affective disorder, migraine headache, morbid obesity, depression and anxiety, kidney stone, chronic back and neck pain, insomnia, sciatica with disc herniation, systolic murmur, opiate dependence, oxygen dependent at home

albuterol sulfate HFA 0.9mg/1 actuation 2 puff prn q4hr, buprenorphine 8 mg sublingual TID, bupropion 150 mg oral daily, gabapentin 600mg oral TID, Iron 27 mg oral daily, Lantus 25 units subcutaneously at bedtime, metformin 1000 mg oral BID

Allergies

SULFA-anaphylaxis, penicillin-hives, metazalone-unknown reaction, lyrica-swelling, cyclobenzaprine-swelling of eyes/anxiety, carisoprodol-unknown reaction, tizanidine-sob/congestion.
