On 15-Apr-2025, the patient was vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II) for prophylaxis (lot #Y015834 was verified to be a valid number, expiration date: 20-Sep-2026) (product quantity: 1 vial) diluted with sterile diluent (BAXTER STERILE DILUENT) (product quantity: 1 prefilled syringe) (lot # and expiration date were not reported) (dose, scheme, strength, formulation injection site and route of administration were not provided). It was reported that when administering Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II) the needle attached to the luer lock on the diluent pre-filled syringe was loose (device connection issue).
