Symptom narrative

Patient called on 5/7/25 at 1023am to report adverse effects from her second shingles vaccine (received on 5/5/25) at the localized injection site (left deltoid) of redness, swelling, and itchiness. She did report that scratching caused it to bleed. Apology was given and was told that we are required to report this incident and to reach out to Pharmacy manager if she has unresolved issues.
