Symptom narrative

Pt. mother was called and informed on 5/1 that there had been a vaccine error. Pt. mother stated during phone call that pt. L thigh was red and swollen around injection site. Mother was advised to bring pt in to be seen. Upon being seen, it was noted by NP that pt. had a 3cm round area at injection site that was warm to the touch. Mother was sent home with antibiotic in the event that area on L thigh did not get better, and was advised to contact PCP NP should it get worse over the weekend.
