Symptom narrative

Pt came in to pick up Rx. We were prompted in our que that she was due for Tdap and HPV. We offered the vaccinations and patient accepted. I pulled database and saw she was due for Tdap and HPV and printed screen. There was no report that it had been given at other location. I gave the shot after confirming multiple times that patient was about to get Tdap and HPV.
