Symptom narrative

Phn went to the refrigerator and accidentally chose Prevnar 15 instead of HPV vaccine to give to patient. Phn realized after giving vaccine what she had done. Patient and patient's mother were informed of what happened and hpv vaccine was then given to patient in the other arm.
