Symptom narrative

There was no adverse effect reported but rather an incorrect vaccine administered to the patient based on their age. Patients guardian brought a list of vaccines requested by the school district. School requested DTap vaccine and Infanrix was given into the patient. The vaccine NDC: 58160-810-52(box) expiration:07/07/2026 and lot number 9KB9G. Dtap was delivered to patient even though they were supposed to receive Tdap based on their age (10 years old). The mistake was realized on 5/8/25 and the patient will be notified via phone call that they can receive Tdap vaccine via VFC program here at our pharmacy.
