Symptom narrative

Patient was seen today 05/08/2025 for 11 y/o well child, patient was brought back, and vitals and physical was done/taken. Patient was only due for HPV, Tdap, and Meningococcal vaccines. Person then drew all vaccine and ordered all vaccines. I helped administer vaccine because person said mom wants both at the same time, I then helped her administer vaccine and caught the mistake once I was entering the vaccine information.
