A 50-year-old female patient (not pregnant) received BNT162b2 omicron (kp.2) (COMIRNATY (2024-2025 FORMULA)), on 03Oct2024 as dose 1, single (Lot number: LM2210) at the age of 49 years, in right deltoid for covid-19 immunisation; influenza vaccine inact sag 3v (FLUCELVAX), on 03Oct2024 as dose number unknown, single (Lot number: 388527), in left arm for immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: VACCINATION FAILURE (medically significant), COVID-19 (medically significant) all with onset 02May2025, outcome "recovering" and all described as "Tested positive for Covid. Experiencing mild symptoms.". The patient underwent the following laboratory tests and procedures: SARS-CoV-2 test: (02May2025) Positive. Therapeutic measures were not taken as a result of vaccination failure, covid-19.
