Symptom narrative

Patient got Arexvy in October of 2023 but this was unknown to pharmacy and patient had forgotten. Presented at pharmacy on 5/14/25 wanting Arexvy vaccine and it was administered after patient verbally stated they had never received one. Discovered after the fact that dose in May was a duplication of the single-dose series. Patient does not report any unexpected side effects at this time.
