Received vaccine 05/13/2025 in the AM. 05/14/2025 AM staff notified us she had area of redness, swelling, warmth, itching, and tenderness. Picture sent to primary care provider who ordered Augmentin 875/125 BID X 7 days.

Cold last part of April

Medical history

Chronic shoulder pain and arthritis, urinary incontinence and overactive bladder, hypothyroidism, sick sinus syndrome, anemia, chronic kidney disease stage 3, CHF, COPD, atrial fibrillation, anxiety, diabetes mellitus, coronary artery disease, hypertension, hyperlipidemia, pacemaker, GERD, obesity.

Other medications

Nystatin Powder TID, Atorvastatin Q HS, Fluconazole Q 3 days, Protonix BID, Synthroid daily, lidocaine ointment BID, tramadol TID, oxybutynin BID. PRN = Tylenol, Ketoconazole, Mylanta, Colace, Senna plus, milk of magnesia, Miralax, Dulcolax
