Symptom narrative

Systemic: Fainting / Unresponsive-Medium, Systemic: incontinent of urine-Mild, Systemic: Flushed / Sweating-Medium, Systemic: Hypotension-Medium, Systemic: Nausea-Mild, Additional Details: Patient became unresponsive, incontinent of urine, eyes rolling and pale approximately 5 minutes after vaccine administration. EMS called and vitals obtained while on call with EMS. Patient unresponsive for approximately a minute. Patient came too and able to answer questions appropriately after but remained, diaphoretic.
