The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant medications included Diphtheria vaccine toxoid;Pertussis vaccine acellular;Tetanus vaccine toxoid (Dtap), Polio vaccine inact (Ipv), and Influenza vaccine. On 17-Mar-2025, the patient was vaccinated with an improperly storage dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Y017511, expiration date: 15-Apr-2026), dose number 1, administered by subcutaneous route in left thigh for prophylaxis, and diluted with sterile diluent (MERCK STERILE DILUENT), administered by subcutaneous route (indication, expiration date, and lot # were not reported). Temperature excursion occurred at -9.8C for 2 hours and 3 minutes. Previous temperature excursion was not reported. No additional adverse event was reported.
