Symptom narrative

Gave 1 dose pediatric dose of Hep A vaccine in error instead of adult dose. Had patient come back in for 2nd pediatric dose in same day to equal an adult dose. He tolerated the vaccines well. Each were given in separate arms. No symptoms or adverse events noted or reported by patient.

Asthma, mild dementia

Rosuvastatin, Lisinopril, Hydrochlorothiazide, Donepezil HCL, Trazadone, Escitalopram, and Albuterol inhaler
