Symptom narrative

Pallor, diaphoresis, nausea, and syncope. Pt assisted to supine position on table and monitored. Pt rested for approximately 5 minutes -10 minutes and stated she "felt okay". Upon standing pt experienced the same symptoms again, assisted to sitting position in chair. Pt experienced syncopial episode. Pt monitored, VS stable, pt responsive after syncopial episode. Symptoms resolved after 15-20 minutes of rest and juice with cracker.
