Symptom narrative

Patient reported approximately 2 weeks after vaccines were given that she experienced shoulder pain. She states she felt this pain 2 days after the vaccines but did not report it until about 2 weeks later.

Patient states she did go to an urgent care center on 4/30/25, 2 days after the vaccines were administered. She did go to the ER later but I am not sure of the date. I do not have any medical reports. I am only going by what the patient is telling me.
