Symptom narrative

Mother reports that at approx. 10 pm she noticed a "rash" in patient's diaper area, that patient was "puffy" all over (child is normally very slim) and mom heard a "weird" sound in her airway, as if she was having trouble breathing therefore she took her to Hospital ER for evaluation.

Lab data

Per ER notes from Dr. reported to this nurse by MA. Dr. noted that child had what appeared to be a diaper rash and that child had no cough during examination. "No indication for emergency transfer or hospitalization." On after visit summary there was a diagnosis noted "vaccination adverse reaction". Nothing else was noted per MA. This nurse plans to reach out to Dr. for clarification when he is available.
