Symptom narrative

15-year-old female was given the HPV #1 yesterday. 20 minutes after she left she reported that she was starting to feel "a little funny". Patient did not say anything. Patient reports later in the day she was changing positions and going from a sitting to a standing position. Patient reportedly lost consciousness and fell back to the ground hitting the front of her head. Patient reports a headache today. Mother shows a picture of bilateral cheeks with a red raised rash. Skin intact on the head. No bruising. Patient does have a headache
