Symptom narrative

5 hours post vaccination, patient developed a red facial rash and had an episode of choking and small spit up of formula. She was assessed in office 1 hour after reaction, 6 hours after vaccines, and facial rash had resolved to mild erythema to forehead. She had normal assessment with clear lungs.
