Symptom narrative

Shingles to underside of right breast with pain to the area noted on 4/25-25 2246. COVID19 vaccine given on 4/21 between 1100-1300. Treated with Valtrex 500 mg TID until 5/3/25

Medical history

Cerebral infacrt of right cerebral artery, hemiplegia, neuralgia and neuritis, atherosclerosis, intermittent claudication bilateral legs,morbid obesity, depression, Fuchs' heterochromic cyclitis bilateral,, hypothyroidism, mild cognitive imparement
