Symptom narrative

Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Medium, Systemic: Flushed / Sweating-Mild, Systemic: Headache-Mild, Systemic: Hypotension-Mild, Systemic: Nausea-Mild, Additional Details: Immediately after needle inserted during COVID-19 vaccination, patient became pale, his head fell backward & he lost consciousness. Vaccine was administered & provider able to keep patient in chair & safe. Patient regained consciousness approximately 10 seconds later after several hard sternal rubs & name shouting. Pt diaphoretic & nauseated with low HR & BP for about 5 minutes; VSS by 0901. Provider had pt call someone to drive him home due to syncope, even though VS had returned to normal
