Symptom narrative

Patient arrived Clinic waiting area via w/c with his wife, c/o sore throat, cough. Wife states he had a Covid vaccine yesterday at the clinic -States after the vaccine, started having trouble breathing. States has productive cough "green" phlegm. Color pale. Wet cough noted. COVID-19 (PFIZER), MRNA, LNP-S, * 1 06/02/2025.

Current illness

current cancer treatment, Hepatic metastasis 8. Bacterial pneumonia R lung , question of aspiration, UTI vs pyleo, and ARF.

Medical history

Hypertension, Diabetes Mellitus, Chronic pain

Squamous cell -tongue metastatic Subjective-he is on hospice since last year in November. With cancer with metastasis.
