Chief Complaint : Administered COVID-19 Vaccine Comirnaty in RUA. Noted that this was expired after it was administered. Patient was notified. Left message on voicemail on 5/23/25 at 12:00.

Medical history

Arthritis, atypical lobular hyperplasia, history of low iron, history of low folic acid, GERD, hypothyroid, malabsorption syndrome, osteoporosis, s/p gastric sleeve procedure, thiamine deficiency, B12 deficiency and vitamin D deficiency.
