Symptom narrative

This vaccine was given in error to patient. Her 16 year old sister was supposed to receive this vaccine and the medical assistant made a gross error in administering this vaccine to the wrong child. This vaccine should not have been administered to a healthy 11 year old with no pre-existing health concerns or need for Men B vaccine. Another form was filled out for her sister, since she received the vaccines meant for patient.
