Symptom narrative

Patient came in to pharmacy on 6/12 at 12:00pm to ask about a reaction she says seems to be due to vaccine. Her arm was red and warm to touch around where pt said injection site was. It was also itchy. Patient said for a few days it was just normal sore but then started getting red, warm and hurting then moving to itchy. suggested pt see doctor incase it is somehow infected cannot guarantee pt went at this point
