Pt had PCV 20 on 6/6/205. Reported noticing pain and redness on 6/12/25. No blisters/pus noted. Pt reported pain and redness improved today (6/13/25). Advised pt to continue monitor sx, advised to use cold compress, return/UCC/ED precautions reviewed, pt v/u.

age-related nuclear cataract of both eyes, dyspareunia in female, hyperlipidemia, dorsalgia, atopic dermatitis, prediabetes, spondylosis of cervical region without myelopathy or radiculopathy, thyroid nodule, radiculopathy of lumbosacral region, DDD lumbosacral, Anisometropia, Dry eyes, calf cramp, mixed stress and urge urinary incontinence, primary hypertension, amblyopia

Other medications

Pt is on rosuvastatin 10mg, cholecalciferol 25mcg, amlodipine 10mg and refresh tears 0.5%.
