Site: Bruising at Injection Site-Mild, Site: Itching at Injection Site-Medium, Site: Pain at Injection Site-Medium, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Additional Details: patient developed redness and swelling- went to urgent care and was prescribed medications for swelling and possible infection- patient is requesting reimbursement for medications filled on 6/03/25 - fluconazole, clindamycin and prednisone
