Symptom narrative

Vaccine administration error. DTaP and polio vaccines were due. Pentacel and polio vaccines were administered. No adverse reaction occurred at the time of administration. The patient's mother reported patient was drowsy and had a decreased appetite on the day of vaccination but returned to her normal activity level the next morning. The patient's mother reported this is common behavior for her after receiving vaccines. As of 06/05/2025 at 12:00 pm, no additional symptoms were reported.
