Symptom narrative

Vaccine for a sibling sitting on the counter next to the patient's vaccine. Two nurses vaccinating patient. Nurse #2 gave the sibling's vaccine to the patient, instead of Gardasil. Patient received 2 doses of pediatric Havrix on the same day. No signs/symptoms of any problems occurred.
