Symptom narrative

Pt was administered Rotavirus. Pt started to vomit after a portion of the dose was administered. Per medical assistant half of the dose was administered. Pt started to vomit x1 and puffy eyes were noted. Upon arrival vitals were monitored. Pulse: 155, O2: 100%. No cyanosis, wheezing per provider. Per provider EMS was called. Pt was taken to Hospital to continue monitoring. Pt was discharged from Hospital the same day.
