On 20-Jun-2025, the patient inadvertently received the adult dose of Hepatitis B Vaccine (Recombinant) (RECOMBIVAX HB) (lot #Y006509, expiration date: 11-Feb-2027) rather than another pediatric dose, 1.0 ml administered as prophylaxis by health professional (accidental overdose).
