Symptom narrative

patient experience nerve pain in shoulder; nerve pain in lower extremities(butt, lower back & legs); difficulty thinking, memory decline; difficulty standing, walking and strength decline; vertigo; pain when attempting to stand; dizzyness; within 1st year after Vaccine and Inpatient hospitalization, prolongation of existing hospitalization, persistent and significant incapacity, substantial disruption of the ability to conduct normal life functions diagnosed to have osteoporosis & dementia declared incompetent all within 36 months after receiving Pfizer Vaccine.
