Symptom narrative

Patient was due for a Hib vaccine, along with PCV20 and Dtap. I drew hib vaccine without realizing powder needed to be mixed. Vaccine diluent was administered by college, while I administered the other two vaccines. Parents were notified of incident, and verbalize patient is well, and denied having no adverse effects. Attempted to schedule patient for a nursing visit to repeat vaccine, but family has moved. Informed parents they should contact us back once they have a new pcp, to address the incident before new Hib vaccine is administered. Parents agreed with plan.
