PT WAS 58 YEARS OF AGE AT TIME OF VACCINE AND DID NOT HAVE A PRESCRIPTION FROM PHYSICIAN, FAMILY HAD REQUESTED VACCINES AT NURSING HOME AND I DID NOT REALIZE HE HAD NO PRESCRIPTION REQUEST.
