Symptom narrative

Pediatric Hepatitis A was given during visit. After administration, it was noticed that the vaccine given was the pediatric dose, rather than the adult dose. About 10 minutes later, patient was given the adult dose of Hepatitis A. No symptoms noted so far. Patient was notified of signs and symptoms to look for, and to contact us if any symptoms occur.
