On 24-JUN-2025, the patient was vaccinated with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) injection, 0.5 ml, lot #Y019465, expiration date: 24-MAY-2026, administered by unknown route as prophylaxis. The vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (no details provided). The vaccine was administered after temperature excursion (Product storage error).
