Product administered to patient of inappropriate age

Symptom narrative

Vaccines prepared to administer and additional staff assisting. Staff member went in the wrong room and administered 1 year old vaccines to a 2 month old. Provider and staff realized error as nurse was leaving the patieint room. Immunization registry contacted immediately. Child was given Tylenol in office and situation was explained to mother. Staff member and provider discussed and there will be a "checks and balances" system in place moving forward in an effort to not have any additional events. Attempted to call this morning and no answer. Voicemail left.
