Symptom narrative

Patient received dose of MenQuadfi given by APRN when this was not indicated, as patient had already received appropriate booster dose at age 16 on 1/13/2020 per submitted immunization records that would meet college health entry requirements. Therefore, patient was given unnecessary dose of a Meningococcal ACWY vaccine. This was discovered on routine chart audit of health records.
