Symptom narrative

Pt/parent report that pt was given 2 meningitis vaccines into upper left arm on 7/3/2025. Pt reports left shoulder started to be painful two hours later, then progressed to decreased ROM and arm spasms when gripping items and continues today. Please note: I cannot confirm the brand name of the vaccines that were given. I was unable to complete form with "unknown vaccine".
